- Select the service that meets the patient’s needs best.
- Prepare the provider and the patient for the change in care,
especially if the patient has dementia.
- Monitor and evaluate the effectiveness of care performed.
- Make adjustments to the care plan or facility/service if needed.
While exploring different services and tour locations, speak in depth with each potential service provider. It is essential that needs are met, and that the individual feels comfortable with the services, provider, and facility. What works for one patient may not work for another. Additionally, as a patient’s needs change, care services may have to be evaluated and altered.
