Description

CoaguChek XS Test Strips

CoaguChek XS PT test strips are single-use reagent strips used with the Roche CoaguChek XS portable coagulometer to measure a patient’s prothrombin time (PT) and INR (international normalized ratio). They are intended for monitoring anticoagulation therapy (e.g. warfarin) in adults (and trained patients) by giving a rapid PT/INR value from a tiny capillary blood sample. Each package contains individually foil-wrapped strips (typically 6 or 24 per box) along with a coded chip (for meter calibration). The strips do not cure any condition – they are a diagnostic tool only.

  • Indications (Uses): Used for PT/INR testing in patients on vitamin-K antagonists (warfarin) or similar anticoagulants. The CoaguChek XS system (strip + meter) quantitatively determines PT (INR) from fingertip capillary blood. It is offered as a convenient point-of-care or home-testing option so patients can regularly check their INR without a venous draw or lab visit. (Note: these strips measure coagulation – they will not detect clotting disorders or any disease on their own beyond INR.)

  • Meter compatibility: Only use with the CoaguChek XS meter. The meter requires inserting a new strip for each test. The strip’s on-board chip automatically codes the meter to that strip batch, so manual coding isn’t needed. The meter then guides the user step-by-step.

How to use (Procedure)

  1. Preparation: Wash and dry hands. Insert a CoaguChek XS strip into the meter’s strip slot (metal contacts first). The meter powers on and displays a prompt (like “Add blood”).
  2. Sample collection: Use a sterile lancet to puncture the side of a fingertip. Collect the first small drop of blood by lightly touching it to the strip’s sample entry port. (Do not use blood drawn into a tube or syringe – tubes contain citrate that will invalidate the test.) The CoaguChek XS needs only a tiny sample (about “8 μL” of blood, roughly one drop). You should apply the blood to the strip immediately (within seconds).
  3. Result: The meter will automatically start timing. Within about 1 minute the INR (or PT) value appears on the display. The result is automatically saved to the meter’s memory.
  4. After testing: Remove and discard the used strip (it is single-use). Cleanse the finger if needed, and proceed with any doctor’s dosing instructions. The meter can be turned off or will auto-shutoff after a brief idle period.

Test specifics

  • Sample: Fresh capillary blood (fingerstick) or non-anticoagulated venous blood. (Never use citrated/frozen samples.)
  • Volume: As little as ~8 µL (eight millionths of a liter) of blood.
  • Range: INR results ~0.8–8.0. (Values outside this range may display as “OL” on the meter.)
  • Accuracy: The system is clinically accurate (about ±5% vs lab INR).
  • Controls: The strip/meter system has internal QC. Users should also run periodic controls (provided with the kit) per instructions to ensure accuracy.
  • Response time: Approximately 1 minute for each result.
  • Storage: Strips are individually foil-sealed with desiccant to protect from moisture. Store the strips in their original container at room temperature (typically 2–30 °C) away from moisture and direct sunlight. Do not freeze or refrigerate, and do not handle a strip with wet or dirty hands. Use strips before their expiration date.
  • Calibration: The CoaguChek XS strips come with a coded chip that automatically calibrates the meter to that strip lot. Always ensure the correct chip (usually already attached to the strips container) is inserted into the meter before testing.

Precautions & Interactions

  • Single-use only: Each strip is for one measurement. Do not reuse strips.
  • Allergy: No ingredients in strips are known systemically active (they are in vitro reagents), so allergic reactions are extremely unlikely. (However, they do contain animal or recombinant proteins – persons with hypersensitivities should exercise general caution.)
  • Inaccurate results: Certain factors can affect INR accuracy. Avoid testing if your hands are extremely cold or sweaty (to ensure good blood flow), and make sure the blood fully fills the strip’s port. Do not use alcohol or iodine with significant blood residue on fingers (it could dilute the sample). Also, high levels of substances like heparin or EDTA (in syringes) will invalidate the test, which is why only fingerstick blood should be used.
  • Control solutions: Use manufacturer-provided controls if needed. If the meter or strip kit has a QC fail message or unexpected result, run a control test.
  • Environmental: The meter and strips should be used within the operating range (generally 18–32 °C and 10–85% humidity [device spec]. Extreme temperatures can affect readings.
  • Storage: Keep sealed strips away from children and pets. Discard strips after expiration date – expired reagents can give falsely high or low INR.

Important Advice

  • Follow medical guidance: Use these strips only as part of a prescribed INR monitoring program. Always review results with your healthcare provider. If an INR result is drastically out of your target range or different from expected, re-test after 5–10 minutes or confirm with a lab test.
  • Proper technique: Ensure hands are warm, do not squeeze the finger excessively (which may dilute the blood with tissue fluid), and fill the strip port rapidly. The CoaguChek XS meter will prompt and guide you through this process.
  • Recordkeeping: Keep a log of your INR results as advised. The meter stores readings, but you may need to report them to your doctor or anticoag clinic.
  • Not a treatment: Remember that these strips only measure INR. They do not treat or change your anticoagulant dose. Dose adjustments should be done under medical supervision based on these results, not on your own.
  • User training: If a caregiver or patient is using the meter at home, ensure they’re trained in the correct technique (inserting strips, finger pricking, reading meter, etc.).
