Pyrantel Pamoate Oral Suspension (Pinworm Medicine)

Pyrantel pamoate is an over-the-counter anthelmintic (anti-worm) medication used to treat pinworm (Enterobius vermicularis) infections. Pinworms are small, thin, white parasitic roundworms that commonly infect the intestines of children (and others). Infection causes anal itching (often worse at night) as female worms lay eggs around the anus. Because pinworm eggs spread easily (via hands, clothing, bedding, laundry), strict hygiene and treating close contacts are important to prevent reinfection. The Centers for Disease Control and Prevention (CDC) notes that pinworm infection is the most common worm infection in the U.S. and is both preventable and treatable.

How Pyrantel Pamoate Works

Pyrantel is a neuromuscular-blocking agent for worms. It paralyzes pinworms in the intestines so they cannot cling and move; paralyzed worms are then expelled in the stool. It works primarily against adult worms (and early stages in the gut) but it does not kill pinworm eggs. For this reason, a second dose is usually needed about 2 weeks later to kill any newly hatched worms. (This two-dose regimen is standard in pinworm treatment.)

Usage and Dosage

Pyrantel pamoate suspension is dosed as a single weight-based oral dose (no repeated daily dosing). The product contains 250 mg pyrantel base per 5 mL (1 teaspoon). Typical dosing is:

  • Children and adults (≥2 years): 5 mg per pound of body weight (11 mg per kg) [pyrantel base] as a single dose, up to a maximum of 1 gram (4 teaspoons). For example, a 50-pound child would take about 7.5 tsp (375 mg) once. The official dosing chart (from the FDA label) provides teaspoon measures by weight.
  • Repeat dose: The dose should be repeated after 2 weeks to kill newly hatched worms (since the first dose does not affect eggs). CDC and other guidelines recommend treating with this two-dose regimen.
  • Household contacts: Because pinworms spread easily, treat all household members (especially children) at the same time, even if asymptomatic. Treating only one person may lead to rapid reinfection from others carrying undetected worms.

Before dosing, shake the bottle well and use a proper measuring device (not a kitchen spoon). Pyrantel suspension can be given with or without food (it may be mixed with juice or given after meals in fussy children). If a child is too young to swallow tablets, the liquid makes dosing easier. Always follow the package directions or your doctor’s instructions for a single dose; do not give extra doses unless instructed.

Efficacy and Supportive Measures

Pyrantel pamoate effectively clears most pinworm infections with the two-dose regimen. It is important to note that medication alone may not break the cycle. Concurrent measures greatly improve success, including: frequent hand-washing (especially after toilet use and before eating), keeping fingernails short and clean, daily morning bathing, and washing bedding/clothes in hot water to remove any eggs. These steps help prevent re-exposure. If symptoms or worms persist after treatment, a healthcare provider should be consulted.

Side Effects and Safety

Pyrantel pamoate is usually well tolerated. Common side effects include mild gastrointestinal symptoms and central effects:

  • Gastrointestinal: Abdominal cramps, nausea, vomiting, and diarrhea. These effects are usually temporary.
  • Neurologic: Headache or dizziness.
  • Allergic reactions (rare): Although very uncommon, some people may have an allergic reaction. Seek medical care if difficulty breathing, hives, swelling of the face/lips/throat, or severe abdominal pain occurs.

Most side effects do not require stopping the medicine and are mild. However, if severe symptoms occur, discontinue use and consult a doctor.

Precautions and Alerts

  • Under age 2: Pyrantel pamoate suspension is not recommended for children under 2 years old except under doctor's advice. (FDA labeling states “Do not use” in those under 25 lbs or under 2 years without physician direction.) Healthcare providers usually avoid routine treatment in very young infants unless the benefits are thought to outweigh risks.
  • Pregnancy: There are no well-controlled human studies. Some experts recommend avoiding pyrantel during pregnancy unless clearly needed. If a pregnant woman has severe pinworm infestation, treatment is generally deferred until after the first trimester, and only given under medical supervision. Discuss with a doctor if you are pregnant or planning pregnancy.
  • Breastfeeding: Pyrantel is poorly absorbed (only ~2–10% absorbed); it is expected to pass into breastmilk in minimal amounts. Most sources consider pyrantel “probably safe” during breastfeeding, but data are limited. Consult a physician before using while nursing.
  • Allergy or liver disease: Do not use pyrantel if you have a known allergy to it (or any suspension ingredient). Also inform your doctor if you have severe liver disease; product labeling advises not to use unless directed by a physician.
  • Drug Interactions: No significant drug–drug interactions are known with pyrantel, but always tell your doctor/pharmacist about all other medicines and supplements you are taking. Unlike some anthelmintics, pyrantel does not typically require dietary restrictions (it may be taken with or without food).
  • Overdose: Serious overdose is unlikely if dosing is done properly. In case of accidental overdose, contact Poison Control or seek emergency care immediately.

Pyrantel pamoate is sold under various brand names (e.g. Reese’s Pinworm Medicine, Pin-X, Pin-Away, etc.) and as generic. The oral suspension strength is typically 144 mg pyrantel pamoate per mL (equivalent to 50 mg pyrantel base per mL). It is available without a prescription in many countries.

Pyrantel pamoate oral suspension is an effective, single-dose pinworm treatment. Follow the weight-based dosing instructions on the label, repeat the dose in 2 weeks, and observe hygiene measures to stop the cycle. Watch for mild GI side effects and use precautions in very young children, pregnancy, or liver disease.
