Jelcaine (Lidocaine 2% Jelly)

Jelcaine Sterile Lidocaine Jelly 2% is an over-the-counter (OTC) topical anesthetic. Its active ingredient is lidocaine hydrochloride 2% (w/v). Lidocaine is a local anesthetic in the amide class. When applied to skin or mucous membranes, it blocks nerve signals (specifically sodium channels) in the area, causing temporary numbness and loss of sensation. In practice, Jelcaine jelly is used to “numb” or relieve pain and itching in minor skin injuries.

Jelcaine is a clear, sterile lidocaine jelly that you apply externally. It acts as a numbing gel for minor pain or irritation.

Indications & Uses:

  • Lubrication and topical anesthesia for:
    • Urinary catheter insertion
    • Cystoscopy
    • Endotracheal and nasogastric intubation
    • Proctoscopy and sigmoidoscopy
    • Gynecological examinations (speculum use, insertions, minor procedures)
  • Burns, cuts, and abrasions for localized numbing and comfort
  • Wound care dressing changes
  • Relief of minor pain, irritation, or itching of the skin and mucous membranes
  • Hemorrhoid pain relief gel and anal procedure anesthesia

In each case the lidocaine gel works by numbing the area so patients feel less pain or itching. (Some stronger formulations of lidocaine jelly are also used by healthcare providers for numbing mucous membranes during procedures, but Jelcaine OTC is mainly for external skin use.)

Mechanism: Lidocaine numbs local tissue by penetrating nerve cells and blocking voltage-gated sodium channels, preventing pain signals from reaching the brain. This onset is usually rapid (within minutes), and the numbing effect is temporary (lasting up to an hour or more, depending on amount applied).

Directions for Use

  • Application: Clean and dry the affected skin first. Apply a thin layer of Jelcaine jelly directly to the affected area. You can use a fingertip or cotton swab to spread it.
  • Frequency: Use no more than 3–4 times per day or as directed on the label. After application, do not cover with bandages or tight dressings unless advised by a doctor, as covering can increase absorption.
  • Age: Approved for adults and children 2 years and older. Do not use on a child under 2 years old without medical advice.
  • Duration: Use only as long as needed. If pain or symptoms continue for more than 7 days, or if they clear up and return within a few days, discontinue use and consult a doctor.

Always read the label for full instructions. Wash your hands before and after use (unless you’re treating your hands). Avoid applying to large areas of body surface or deep wounds.

How Does Jelcaine Lidocaine Jelly 2%  Work?

Jelcaine Lidocaine Jelly 2% works by blocking nerve signals in the skin and mucous membranes. The result: rapid, lasting numbness and relief from pain or discomfort during a variety of clinical and minor surgical procedures. Its sterile formulation further reduces the risk of infection and is gentle on sensitive areas.

Precautions and Warnings

  • External use only: Jelcaine is only for external application on skin. Do not swallow, and avoid contact with eyes, ears, nose or mouth. If the jelly accidentally enters a sensitive area, rinse thoroughly with water.
  • Avoid sensitive areas: Do not apply to eyes, open sores (deep cuts), raw surfaces or blistered skin. Also avoid use on large areas of broken skin unless directed by a healthcare provider.
  • Allergy warning: Do not use if you have a known allergy to lidocaine or other amide-type local anesthetics. As with any topical medication, some people may develop skin allergies. Stop use immediately if you notice signs of an allergic reaction, such as skin rash, itching, hives, swelling of the face or throat, or difficulty breathing. Seek medical help if these occur.
  • Other medical conditions: Inform your doctor or pharmacist if you have G6PD deficiency (an enzyme or blood condition), heart disease, high blood pressure, liver/kidney disease, or any blood disorders. (Lidocaine can very rarely cause a blood condition called methemoglobinemia, especially in G6PD-deficient individuals) If you are pregnant, planning pregnancy, or breastfeeding, consult a healthcare professional before using.
  • Drug Interactions: After topical use, lidocaine can be absorbed systemically. Be cautious if you are using other medications—especially other local anesthetics or certain anti-arrhythmics—since combined effects can increase risk of toxicity. Always tell your doctor/pharmacist about all other medications or supplements you use.
  • Children and elderly: Use with care in the elderly or in very young children. Children may be more susceptible to side effects. If using on a child (2–12 years), follow the label and consider consulting a pediatrician. Gently test a small amount on normal skin and wait to see if any reaction occurs before general use.

Storage: Store at room temperature (68–77°F or 20–25°C) away from heat and light. Keep the tube tightly closed and out of reach of children.

Side Effects

When used as directed on intact skin, lidocaine jelly is generally well tolerated. Common side effects are usually mild and localized to the application site:

  • Local effects: Mild burning, stinging, redness, swelling, or irritation at the site of application. These are often temporary and diminish quickly. Dryness or tenderness may also occur.
  • Systemic effects (rare): If too much lidocaine is absorbed (for example, from applying on large areas of broken skin or swallowing it), it can cause central nervous system or heart effects, such as dizziness, confusion, headache, tremors, or rapid heartbeat. These situations are uncommon with normal use.
  • Allergic reaction: Although rare, some people may have hypersensitivity. Signs include severe rash, hives, or swelling of face/lips, which require immediate medical attention.
  • Methemoglobinemia (very rare): Lidocaine can rarely cause this blood condition, which impairs oxygen delivery. Warning signs include pale/blue skin (especially lips and nails), shortness of breath, very rapid heartbeat, or unusual tiredness. Seek emergency care if these occur.

If any side effect is severe or persists, stop using Jelcaine and contact a doctor or pharmacist. In case of accidental overdose (such as ingesting a significant amount), call Poison Control or emergency services right away.

Important Notes

  • Not a treatment substitute: Remember that Jelcaine only provides temporary symptomatic relief of minor pain/itching. It does not cure the underlying injury or condition. If the wound is large, deep, or severely burned, seek medical care instead of relying on topical anesthetic.
  • Keep out of children’s reach: The tube is not child-resistant. If a child accidentally swallows it, get medical help immediately.
  • Pregnancy/Breastfeeding: Use with caution. Discuss risks and benefits with your healthcare provider. Lidocaine does pass into breast milk in small amounts, so nursing mothers should use it moderately or pump-and-discard as advised by a physician.

Always follow clinical protocol and safety guidelines. Not for ophthalmic use. Seek physician advice for children or in case of allergies or adverse reactions.
