Ondansetron 4 mg Tablets – Dosage Information

Ondansetron (Zofran) 4 mg tablets are oral anti-emetic tablets used to prevent nausea and vomiting from chemotherapy, radiation therapy, and surgery. The dose depends on the situation (see prescribing information). Key dosing regimens are:

  • Chemotherapy-induced nausea/vomiting (CINV) – adults:

    • Moderately emetogenic regimens: 8 mg orally 30 minutes before chemotherapy, then 8 mg again 8 hours later, then 8 mg every 12 hours for 1–2 days after chemotherapy. (Each 8 mg dose is two 4 mg tablets.)
    • Highly emetogenic regimens (e.g. high-dose cisplatin): 24 mg orally once, 30 minutes before chemotherapy. (six 4 mg tablets).

These serious reactions are rare, but ondansetron can prolong the QT interval on an ECG, so any chest pain, fainting or palpitations should be evaluated immediately.
